ATTENTION PARENTS/GUARDIANS OF STUDENTS WHO WILL BE IN GRADE 7 FOR THE 2025-26 SCHOOL YEAR! MANDATORY: Grade 7 Immunization & Physical Exam

The following documentation should be submitted to the school prior to the start of the 2025-2026 school year to meet the state/district 7th grade requirements. Please ensure that you have an appointment to get this completed and submitted.
IMMUNIZATIONS
Florida Certification of Immunization form DH 680
- Hepatitis B - alternate two-dose series for adolescents 11-15 years of age; both two-dose and three-dose series require a minimum of 4 months to complete.
- Two doses Measles, Mumps and Rubella Vaccine (MMR) - (The second dose of MMR vaccine was recommended beginning school year 2008-2009; however, students already enrolled in 7th Grade with a valid DH680 having two measles, one mumps, and one rubella immunizations should not be excluded. They should be advised to receive a second dose of MMR).
- Two doses of Varicella (chicken pox) - Varicella vaccine is not required if the child has documentation of history of varicella disease.
- Tetanus/Diphtheria/Pertussis (Tdap) booster
SCHOOL PHYSICAL EXAMINATION

The Physical Examination must be completed within 12 months prior to the date of entry into 7th grade. Physicals MUST have been conducted after 8/12/24. Make sure that all sections/questions are completed and ensure the parent signs on Page 1 and the physician sign/stamp on Page 2!
ONCE YOU HAVE BOTH COMPLETED FORMS (make a copy), then PLEASE SUBMIT THE FORMS IN ONE of the FOLLOWING WAYS:
- Scan, Attach, and Email Karyna Kanhoye the physical & immunizations forms. You must include the following in the email: parent’s full name, student's full name, student I.D., grade level.
- Mail the physical and immunizations forms and include the following: parent’s full name, your child's full name, Student I.D., and grade level.
